Connecting Sontek Residents with the Right Services, Organizations and Community Support

The Community Resource & Referral Program of Youth Advancement Sontek Society (YASS) is designed to help individuals and families in the Sontek community identify appropriate services, understand available community resources and connect with organizations, professionals, government offices and service providers that may be able to address their needs.

Community needs can be diverse. A resident may need information about a healthcare provider, while another family may require educational support, employment services, childcare information, transportation assistance, housing resources, legal guidance or access to a government service. In many situations, appropriate services may already exist, but residents may not know where to find them, which organization to approach, how to contact the service or what steps should be taken next.

Through this program, YASS provides a structured community-based process involving needs identification, resource search, service information, referral, connection with appropriate providers and follow-up where necessary.

YASS does not replace professional healthcare providers, lawyers, financial professionals, government authorities, educational institutions or other specialized service providers. Where professional or specialized assistance is required, YASS's role is to help residents identify and connect with an appropriate resource or referral pathway.

Making Community Services Easier to Find, Understand and Access

A community may have many useful services and organizations, but those resources are only helpful when residents know what is available and how to access it.

Residents may sometimes spend considerable time approaching different offices or organizations without knowing whether those services are appropriate for their circumstances.

For example, a resident experiencing an employment difficulty may need an employment-support organization rather than financial assistance. A family requiring childcare may need information about available childcare providers. A person experiencing a legal issue may require a qualified legal service. An older resident may need transportation assistance or information about a healthcare facility.

YASS therefore acts as a community connection and referral point, helping residents identify suitable options and navigate toward the appropriate organization, service provider or professional.

The purpose is to develop a clearer understanding of what type of community resource, service provider or professional may be appropriate. This is a community-support assessment and does not replace professional, medical, legal, financial or clinical assessment.

The Community Resource & Referral Program is primarily intended for individuals and families living in or connected with the Sontek community who need help identifying, understanding or connecting with appropriate community resources and services.

Receiving a referral from YASS does not guarantee acceptance, eligibility, appointment availability, financial assistance or a particular outcome from an external organization.

YASS may review whether the resident successfully contacted or accessed the recommended service and whether further assistance is needed.

You do not need to know exactly which organization, professional or service provider you should approach before contacting YASS.

YASS can help you understand your needs, identify potentially relevant resources, explore available options and connect with an appropriate service or referral pathway.
