The Community Health & Wellness Program of Youth Advancement Sontek Society (YASS) is designed to help Sontek residents improve their understanding of everyday health, preventive practices, healthy lifestyles, personal wellbeing and available healthcare resources.
People may experience difficulty understanding health information, identifying appropriate healthcare services, maintaining healthy everyday routines or knowing where to seek professional assistance when a health concern arises. Some residents may also have limited opportunities to participate in community health-awareness activities or preventive screening initiatives.
Through this program, YASS organizes and coordinates health-awareness activities, wellness education, community health events, healthy-living initiatives, general preventive-health information and connections with appropriate healthcare providers and services.
Where possible, YASS may collaborate with qualified doctors, nurses, healthcare institutions, diagnostic centres, public-health professionals, NGOs and other appropriate organizations to deliver community health activities requiring professional expertise.
The program does not provide medical diagnosis, prescribe medication, replace professional medical treatment or act as a healthcare facility. Residents requiring medical assessment or treatment should be referred to an appropriately qualified healthcare provider.

Building a Healthier and Better-Informed Community
Good community health involves more than responding to illness. Residents can also benefit from understanding preventive health practices, nutrition, hygiene, physical activity, available healthcare services and when professional medical attention should be sought.
Different residents may have different health-related information and support needs. A young person may benefit from nutrition or hygiene awareness. A parent may need information about maternal or child-health services. An older resident may need assistance identifying an appropriate healthcare provider. Another resident may simply want to participate in a community health-awareness event or screening program delivered by qualified professionals.
YASS therefore provides a community-based platform through which residents can access general health information, participate in appropriate wellness activities and connect with professional healthcare resources when necessary.

Where professional assessment or treatment is necessary, YASS can help identify an appropriate referral pathway.
