Helping Sontek Residents Understand Their Needs, Find the Right Support and Move Forward

The Family & Resident Support Program of Youth Advancement Sontec Society (YASS) provides a structured and approachable point of support for individuals and families in the Sontek community who may be experiencing social, family, educational, employment, financial, housing, health-related or everyday practical difficulties.

Residents do not always know which organization, service provider, government office or community resource can address a particular problem. In other situations, a resident may have several needs at the same time and require assistance identifying which issue should be addressed first.

Through this program, YASS helps residents discuss their circumstances, identify needs and existing strengths, understand available options, develop practical support plans, connect with appropriate programs and service providers, and receive follow-up support where necessary.

The objective is not to replace professional healthcare, legal, financial, counselling or government services. Instead, YASS works as a community-based support and navigation point, helping Sontek residents reach appropriate resources while providing suitable non-clinical community support.

2. What the Program Includes

Resident Consultation & Office Visit

Understanding the resident's situation before determining next steps.

The initial contact helps YASS understand the resident's situation before determining the appropriate next step. This may include gathering relevant information about:

Current circumstances
Family or household situation where relevant
Education and employment status
Immediate concerns
Existing community or family support
Services already being accessed
Reason for approaching YASS
Type of assistance being requested

Only information reasonably necessary for providing or coordinating support should be collected.

3. Who Can Participate?

The Family & Resident Support Program is primarily intended for people living in or connected with the Sontek community who require information, guidance, practical community support or assistance connecting with appropriate resources.

Eligible Participants

Eligible Participants — This may include:

Individuals seeking general community support
Families experiencing temporary difficulties
Parents or guardians seeking relevant resources
Young adults requiring additional guidance
Unemployed or underemployed residents
Older residents
Persons with disabilities requiring community-resource information

People requiring education, employment or training referrals
Residents experiencing housing-related difficulties

Support-Specific Requirements

Some services or external programs may have their own eligibility, age, income, documentation, residency or other requirements.

Participation in YASS does not guarantee acceptance into an external program or entitlement to a government benefit.

Residents who may experience greater difficulty navigating services can receive additional appropriate community assistance.

Continued Follow-Up

Where necessary, support does not have to end after the first conversation. Progress can be reviewed and further options identified.

Greater Independence

The long-term objective is to help residents better understand available resources and become increasingly capable of navigating appropriate services independently.
